Elisa Mahlberg, gynecologist in Dormagen – contraception advice

Contraception is a personal topic that shifts several times over a life. First contraception at 16, after giving birth, in your early 40s, before or after a child — each phase may call for a different answer. In our contraception consultation you get a comparison of methods that fits your life situation.

Talking through contraception calmly

There are many half-truths about contraception — from "the new pill is the best" to "the IUD hurts". And yet this is a decision that shapes your everyday life, your body, often your mood. We go through the methods with you so that by the end it is clear what fits medically and what feels right for you personally.

We explain how each method works, what realistic side effects look like, what the real failure rates are, and what makes sense in your current life phase. The decision is yours.

What a contraception consultation with Elisa Mahlberg looks like

A consultation typically runs as follows:

  • Talking through your situation. Age, cycle, partnership, previous contraception (if any) and your experience with it, short- or mid-term fertility plans, whether you smoke, relevant conditions such as thrombosis, migraine with aura or hormone-dependent tumors.
  • Comparing the options. We go through the realistic methods – pill, hormonal IUD, copper IUD, subdermal implant, vaginal ring, patch, condom, NFP-based methods – and explain mechanism, Pearl index, side-effect profile and daily practicality.
  • Gynecological exam and ultrasound if relevant. For example, before fitting an IUD.
  • Decision or time to think. You do not have to commit during the appointment. Many women take the information home and decide calmly – that is absolutely fine.

Contraception options compared – hormonal, hormone-free, NFP

Our contraception consultation covers every relevant method and explains it honestly.

  • Hormonal methods: combined pill, progestin-only pill, vaginal ring, patch, hormonal IUD, subdermal implant. Mechanism, risks (e.g. thrombosis risk depending on the preparation), side-effect profile — all transparent.
  • Hormone-free contraception: copper IUD, condom, diaphragm. For women who want to stay off hormones.
  • Natural family planning (NFP): sympto-thermal methods, cycle apps with and without measuring – we explain strengths and realistic failure rates.
  • Sterilization and partner methods: where a surgical consultation makes sense and who we refer to.

Our scope: we do not fit the copper chain (Gynefix). If this method is a candidate for you, we refer you to a practice specialized in it. The classic copper IUD and hormonal IUD we fit ourselves – details on our IUD fitting page.

The best contraception is the one you can actually live with. Everything else is theory.

When is the right time for a contraception consultation?

An appointment makes sense if any of the following apply:

  • You are considering the topic for the first time and want to know which method fits you,
  • you want to come off the pill and figure out what comes next,
  • you need a new method after birth or during breastfeeding,
  • you are dissatisfied with your current method (side effects, handling, safety),
  • you want something hormone-free or generally less continuous hormone,
  • you are thinking about an IUD and want to know whether it fits you,
  • you are over 40 and wonder how long contraception is still needed.

You do not need to know which method you want before coming – that is the purpose of the appointment.

FAQ

Frequently asked questions about contraception advice

We discuss all common methods: hormonal (pill, progestin-only pill, vaginal ring, patch, hormonal IUD, subdermal implant), hormone-free (copper IUD, condom, diaphragm) and natural family planning (NFP, sympto-thermal, cycle apps). We explain honestly what is medically sensible for your life phase and medical history – and what is not.

The most reliable hormone-free methods are the copper IUD and – when used consistently – combinations of NFP (cycle tracking with temperature and cervical mucus) plus barrier methods. We go through both concretely in the consultation. Condoms are also the only protection against sexually transmitted infections.

There is no single "right" way for everyone. We are happy to discuss with you why you want to come off, what contraception might make sense afterwards, what you expect from your body, and whether known topics like PCOS or endometriosis need to be considered.

For patients with German statutory health insurance, the contraception consultation is generally a covered benefit. Certain contraceptives and their fitting — for example an IUD — can become self-pay services from age 22. The costs are discussed transparently beforehand.

No. The copper chain is a special variant anchored into the uterine wall. We do not offer it in our practice. If this is a candidate method for you, we refer you to a practice specialised in it. The classic copper IUD and the hormonal IUD we fit ourselves.

The usual guidance: for women up to 50, roughly two years after the last period; from 50 onwards, about one year. We are happy to discuss the right method for this stage in our menopause consultation.
