IUD fitting – with information and a gentle approach

Many women interested in an IUD are apprehensive about the fitting. That is understandable: it can be uncomfortable. The entrance to the uterus is narrow in women who have not given birth, and short, sharp pain or a strong cramp during the passage through the cervix is part of the experience. But it does not have to be the whole story.

In our practice we prepare the fitting of a hormonal or copper IUD together with you. We explain the procedure, ask about your pain history and discuss what you would like in terms of pain management.

How an IUD fitting with Elisa Mahlberg is done

We first talk through together which IUD fits your situation — hormonal or copper. Before the fitting, a gynecological examination with ultrasound is performed and the uterus is measured. The IUD is then inserted through the cervix into the uterus. We confirm the position with ultrasound afterwards. The right timing for the fitting and all important notes for before and after the appointment are discussed individually with you.

A position check follows 4 to 6 weeks later. After that, we check the position during routine care or if you have any complaints.

Pain-management options for IUD fitting

To help make the fitting well tolerated, two options are available in our practice and we discuss them with you individually beforehand: medical preparation of the cervix to soften it (cervical ripening with misoprostol, off-label use) or local anesthesia at the cervix (with lidocaine). Which approach fits you — either option alone, both combined, or none — we decide together in conversation before the fitting.

Alongside that: calm positioning, unhurried pace, practiced technique. The pain-management approach is discussed with you individually before the fitting.

Our scope: we do not offer IUD fitting under general anesthesia. If you would like a general anesthetic (e.g. for severe anxiety, a history of trauma, or anatomical reasons), we refer to a gynecology practice in Düsseldorf with a corresponding hospital cooperation. Follow-up afterwards stays with us.

When is the right time for an IUD?

The IUD fits many life phases: after completing family planning, after a birth, after breastfeeding, after coming off the pill, in your late 30s and 40s – or simply because you want a method you do not have to think about every day.

  • The hormonal IUD works for 3–8 years (depending on model) and reduces the menstrual flow for many women.
  • The copper IUD works for 3–10 years (depending on model), entirely hormone-free. The period remains or can become slightly stronger.

Fitting is best done at the end of a period or just after. If you are unsure which model fits, start with a consultation.

The cost is split between the IUD itself and the medical fee and depends on the chosen model. For patients with German statutory health insurance up to age 22, contraceptives and their fitting are covered. From age 22 onwards, this is generally a self-pay service — the exact cost for your situation we discuss transparently with you in advance.

It varies. Most women describe a strong cramp for a few minutes – comparable to severe period pain. Women who have not given birth often feel it more intensely. For that reason we discuss with you individually before the fitting how to make the procedure as well-tolerated as possible — for example through medical preparation of the cervix (cervical ripening with misoprostol) or local anesthesia (lidocaine).

Yes. Helpful: a painkiller taken in advance after consultation with us; calm breathing; no tight schedule afterwards — give yourself the afternoon at home with a hot water bottle. If a cervical preparation is planned, you will get precise instructions for it.

The hormonal IUD releases a progestin locally into the uterus. There are different models with a duration of 3 to 8 years. The flow usually becomes lighter; often periods stop entirely. Which model fits your situation, we discuss individually. The copper IUD works hormone-free via copper ions. Periods remain and can become somewhat stronger or crampier. Lasts 3–10 years. Both are highly reliable (Pearl index usually below 1).

No. In our practice we fit IUDs as an outpatient procedure with gentle pain management. If you need or prefer general anesthesia, we refer you to a gynecology practice in Düsseldorf with a corresponding hospital cooperation. Follow-up afterwards stays with us.

After fitting, the first check-up is at the end of the next period (about 4–6 weeks) – a position check by ultrasound. Later, annual routine checks or on symptoms. We will tell you about warning signs that mean you should come back earlier – missing thread, strong pain, breakthrough bleeding.
