Cancer screening at the gynecologist follows clear guidelines in Germany. You get the statutory screening programme in the gynecological scope, and on request we complement it with individual self-pay services such as breast ultrasound and transvaginal ultrasound. Which examinations make sense in your situation, we discuss together.

The annual check-up: examination, counsel, framing

The annual cancer screening is a good occasion to bring up topics that otherwise easily fall through the cracks — cycle, changes in the breast, contraception, menopause, personal observations. Calmly, in a medical examination, with clear framing.

What happens in a gynecological cancer screening

Our cancer screening is age-based and follows current German screening guidelines:

  • From 20 years: history, gynecological exam, pap smear of the cervix, guidance on breast self-examination.
  • From 30: additional manual breast examination.
  • From 35: instead of annual pap, a combined screening (pap + HPV test) every three years – per current guideline.

Colorectal cancer screening is also an important part of general preventive care. We mention it when relevant and advise when further work-up — for example a colonoscopy — makes sense. The procedure itself is performed by your GP or a gastroenterology practice.

Pap smear, breast ultrasound, transvaginal ultrasound – what we add

Alongside the statutory screening we offer additional examinations where they make sense in your situation:

  • Breast ultrasound (mammasonography). Radiation-free examination that can complement palpation — for example in women with dense breast tissue or as individual-screening IGeL service. With a medical indication (e.g. unclear palpation, family history) the examination is covered by statutory health insurance.
  • Transvaginal ultrasound. Detailed view of uterus, ovaries and endometrium. As individual screening, an IGeL service. With a medical indication (e.g. cycle disorders, menopause follow-up, family history), covered by statutory health insurance.
  • HPV test. Part of the statutory combined screening from age 35; earlier or on request possible as IGeL.

Before any IGeL service we transparently discuss what to expect, what it contributes and what it costs. If a service is not meaningful for your situation, we say that just as clearly.

When is the right time for cancer screening?

The gynecological check-up is an annual appointment linked to your age. Regardless of age, please come outside the regular rhythm if:

  • you have newly palpated something in the breast,
  • you notice irregular or persistent bleeding outside your period,
  • you have new pelvic symptoms that do not go away in a few days,
  • you have symptoms during sex that are not cycle-related,
  • you have a family history of breast or ovarian cancer and want to frame it.

Uncertainty is a good reason for an appointment. When in doubt, we prefer to look earlier.

Frequently asked questions about cancer screening

By age: history and gynecological exam, pap smear from age 20, additional manual breast examination from 30, combined pap and HPV screening every three years from 35. On request we add breast ultrasound and transvaginal ultrasound. We also bring up colorectal cancer screening when relevant and advise you on further work-up via your GP.

Usually not — more briefly uncomfortable. If you have had poor experiences in the past, please tell us beforehand. We will be particularly gentle.

Both are IGeL services under statutory insurance if there is no medical indication. With a medical indication (unclear palpation, cycle disorder, family history) they are covered by statutory insurance. We discuss the cost of any IGeL service with you transparently before the appointment.

Since 2020, Germany uses organised screening: from 35, a combined pap + HPV test is performed every three years. It is more sensitive and detects relevant changes more reliably than annual pap alone. The other components (examination, conversation) remain annual.

Up to 34: annually with pap. From 35: annually for the remaining components, plus pap + HPV every three years. With previous abnormal findings, intervals may be shortened individually – we discuss this in person.

In principle yes, but a pap smear during the period is less informative – so we prefer to schedule mid-cycle. If your period arrives unexpectedly, let us know beforehand and we will reschedule or adjust the appointment.
